Select a chronic disease for a patient teaching product. Submit your choice to faculty for approval.
Identify a patient with a chronic disease for your project.
Create a patient profile for this individual using the Blank Patient History & Physical Form found on the student web site.
Select an educational strategy for your chosen patient. Include the following:
•	The incidence of the disease
•	Typical presentation of the disease
•	The physiological, psychological, and economic effect
•	Specific socio-cultural aspects of the disease
Create an appropriate educational product for your patient.
•	Identify potential learning implications using Bloom- taxonomy.
•	Defend teaching strategies in light of literacy levels, age, and cultural indicators.
•	Describe materials used and rationale.
•	Identify community, state, and national resources.
•	Compare conventional, complementary, and alternative therapies available.
Note: you may submit a video, a public service announcement, role playing scenario, or a PowerPoint presentation. Discuss appropriate deliverable with your faculty member.
Include speaker notes, an outline of the educational product, and at least five references.
