following scenario.
Your hospital
has learned that multiple patients with a diagnosis of congestive heart failure
are being readmitted (return to the hospital for additional services) 28
percent of the time. You have received notification that CMS (Medicare) is no
longer going to reimburse hospitals for readmissions within 30 days.
Your task for this presentation is to provide the
following:
Outline a strategy to tackle this issue.

Discuss the following and how you identify and address/implement
changes:
o Is it due to lack of
quality care by the physician?
o Is it due to lack of
patient education?
o Is it due to lack of
patient aftercare resources?
o Could it be attributed to
other factors-identify please?

You should consider and present which quality methods and tools you
might choose to implement.

Use IOM’s six aims as a framework and describe how you would use each.
·
Include how healthcare technology and the HITECH Act will play a role in
this improvement effort.
Requirements:

Prepare a narrated video presentation using Zoom Pro and presentation
software of your choice (PowerPoint, Keynote, or Prezi, etc.)
o Include comprehensive
speaker notes.
o The recording is to be
stored to the Zoom cloud. You will receive an email containing the link to the
recording so that you can submit it as part of your assignment.
o The presentation should
include a title, the content and reference slides.
o The recommended length of
the narrated video is 10 minutes.
