Introduction

Health literacy continues to be a challenge in population health and adults routinely have difficulty using health information that is readily available to them in retail stores, healthcare provider offices, and within the community. Some level of targeted and tailored messaging needs to be created for most people to understand.

Your clinic uses a patient portal but utilization is low.  Your portal has obesity specific information that patients can access.  It is important that patients understand what obesity is and how access to information could benefit healthy lifestyles. You are tasked with creating a memo that patients will receive upon check out from the clinic, explaining to them why they should utilize the portal to review this information.

Requirements

·         This report should be prepared as a Microsoft™ Word document, and then attached to the unit discussion thread. There is no minimum or maximum in terms of the word count; however, the response should explicitly address all required components of this discussion assignment.

·         The document should not be prepared consistent with the APA writing style (6th edition) except that the in-text citations (if applicable) and references should be properly formatted.

1.      Initial Post: Based on their observations, in this scenario, and upon a careful review of the available literature, the student is to consider him or herself to be health coordinator for a primary care clinic. 

1.      The student is to prepare a formal memo which will go out to patients on the importance of utilizing the portal to access important information on obesity prevention.
