The patient- history is one of the most important aspects of this process. Using the resources available for this unit and any other resources you may find, explain what the role of a patient- history is in determining the correct evaluation and management code. In your answer, be sure to list and explain the four elements of a patient history and give at least one example of what might be found in each element.
